About our insurance plans and payments
Initial visit
Please provide us with current identification, insurance card and billing information if you would like us to bill your insurance for you. Payment is due at the time of visit. We accept cash, checks, Visa, Mastercard, and Discover.
Health insurance
We will bill your insurance company as a courtesy to you. Each insurance company has its own rules for determining how much they will pay on each claim. We will try to answer any questions you have about your insurance, but your policy is a contract between you and your insurance company. It is your responsibility to know your insurance policy and be familiar with your coverage. You should contact your insurance company if you have questions regarding coverage. If your insurance company denies your claim you are responsible for payment in full.
Intermountain Eye Centers prepare and submit insurance claims to your primary carrier. The amount the insurance company will pay is determined by your policy. However, you are responsible for making payments on your bill regardless of your insurance situation. You will receive a monthly statement from our clinic as long as your account is active.
Intermountain Eye Centers participates with some vision plans. Just because you have a vision plan, does not mean that your vision insurance will be billed for your eye exam. Vision insurance only covers non-medical ‘routine eye exams’ relating to glasses and contact lenses. If you have any ocular medical condition or diagnosis, your medical insurance will be billed.
